Persistent dry skin
Most dry skin is not a blood-test problem. Cold weather, low humidity, hot showers, harsh soap, and the natural thinning of the skin's barrier with age explain the large majority, and those respond to gentler bathing and moisturizer, not a lab panel. Testing matters for the minority of cases where dryness is a signal of something systemic.
When dry skin is worth testing
The honest starting point is that everyday dry skin is environmental and structural. The outer skin loses water faster than it can hold it in cold, dry, over-washed conditions, and with age it makes fewer of the lipids and natural moisturizing factors that keep it supple. None of that shows up on a blood test, and none of it needs one.
Blood testing earns its place when dryness is sudden, severe, widespread, unresponsive to moisturizer, or paired with other symptoms like fatigue, itch without a rash, thirst, or weight change. In those cases it can be a window onto the thyroid, blood sugar, the kidneys, or a nutritional gap. Below, each systemic contributor is paired with what the research actually found, and flagged honestly, so you can see which cases warrant a panel and which just need a good moisturizer.
What drives it, and what the evidence shows
1. Environment and the aging skin barrier
This is by far the most common cause and it is not a blood-test problem. Cold air, low humidity, forced-air heat or air conditioning, hot showers, and harsh soap strip the skin's water faster than it can be replaced, and with age the skin makes fewer barrier lipids, so the same exposures dry it out more easily.
Dry skin is a common condition in older adults, but it is not a normal, inevitable part of aging.

2. An underactive thyroid
Thyroid hormone drives skin-cell turnover, barrier-lipid production, and sweat and oil output. When it runs low, dead cells build up and the barrier weakens, producing dry, rough, scaly skin, usually alongside fatigue, cold intolerance, and hair thinning. It is the top testable cause.
Dry, rough skin is the most common skin change in an underactive thyroid, appearing in over half of patients in one large series.

3. Diabetes and high blood sugar
Chronically high glucose pulls water out through the urine, damages the small nerves and vessels that keep skin hydrated, and stiffens barrier proteins. Dry, cracking skin, especially on the feet, is a common finding and a foot-ulcer risk factor.
Among 750 people with diabetes, dry skin was present in about 26 percent, one of the most frequent skin findings.

4. Chronic kidney disease
Failing kidneys let waste products build up and disturb calcium, phosphate, and the sweat and oil glands, leaving skin markedly dry and often intensely itchy. Dryness is one of the most common skin findings in dialysis patients.
Among 100 hemodialysis patients, dry skin was the single most common finding, present in 79 percent.

5. Iron deficiency
Iron powers the enzymes that build and renew skin, so low iron can thin the skin and weaken the barrier, contributing to dryness and sometimes itch, alongside more classic signs like brittle nails and hair shedding.
Iron is among the minerals whose deficiency can show up in the skin, hair, and nails, with dry skin a recognized sign.

6. Zinc deficiency
Zinc is essential for skin-cell division and barrier repair, and severe deficiency causes a dry, scaly, eczema-like dermatitis, classically around the mouth, hands, and groin. It is a real but uncommon cause, usually from malabsorption, a very restrictive diet, or a genetic transporter defect.
Severe zinc deficiency produces a scaly dermatitis, mirrored in zinc-deficient animals that develop the same condition.

7. Vitamin A deficiency
Vitamin A regulates normal skin-cell maturation, and deficiency can cause dry skin with rough, goosebump-like follicular plugs. But this is rare in developed countries and, in practice, usually tracks with broad malnutrition rather than an isolated low vitamin A.
Rough, dry follicular skin is not reliably explained by vitamin A deficiency alone; general malnutrition is the stronger link.

8. Menopause and estrogen decline
Estrogen supports skin collagen, oil production, and water-holding capacity, so as it falls around menopause the skin loses moisture and thins, and dryness becomes common. This is usually judged by menopausal status rather than a blood test.
After menopause, estrogen loss is associated with skin dryness, thinning, fine wrinkling, and poorer healing.

How dry skin is actually evaluated
The first question is whether this even needs a lab. For typical dryness, worse in winter or after hot showers and responsive to moisturizer, the answer is usually no. Better humidity, gentler bathing, and a good moisturizer address the cause directly, and a normal panel would not change that.
When dryness is sudden, severe, widespread, stubborn despite moisturizer, or paired with other symptoms, a physician reads a focused panel: thyroid, blood sugar, and kidney function cover the main systemic causes, with iron and, rarely, zinc or vitamin A added when the history points that way. So testing here is targeted, meant to catch the minority of cases where the skin is signaling something deeper, not a routine step for everyone with a dry patch.

- Intense, persistent itch with no visible rash
- Yellowing of the skin or the whites of the eyes
- Excessive thirst, frequent urination, and unexplained weight loss
- Dry, itchy skin with known or suspected kidney disease, swelling, or reduced urine output
- New, severe, or rapidly spreading scaling or cracking, especially with fatigue, cold intolerance, or hair loss
Ordinary dry skin from cold, low humidity, or over-washing usually responds to gentler bathing and moisturizer, not a blood test. The signs above are worth a prompt visit rather than more moisturizer.
