We pair compounded medications (prepared by FDA-registered 503A pharmacies) with ongoing physician oversight from licensed U.S. providers, shipped directly to you in all 50 states.
Yes. Telemedicine services are provided by Telegra MD, a U.S. physician-led telehealth services provider. Every patient is reviewed by a licensed U.S. physician who is responsible for prescribing and ongoing care. OPTML is LegitScript Certified as a healthcare merchant.
Licensed U.S. physicians within the Telegra MD network. The same physician reviews your intake, your labs (or orders new ones), determines whether you're a candidate, and remains your provider for ongoing dose titration and messaging.

Three steps: (1) Complete the free assessment — about 4 minutes. (2) A U.S.-licensed physician reviews your intake and (if appropriate) lab work, then designs your protocol. (3) Your medication ships in 2–3 days in temperature-controlled discreet packaging. No charge until the physician confirms you qualify.
It depends on the protocol. TRT, HRT, NAD+ and sermorelin typically require baseline labs. GLP-1 weight management can usually start without new labs if you have recent results within 12 months. If you don't have labs, your provider can order them through any Quest or Labcorp in the country.
The initial questionnaire takes about 4 minutes. Physician review is usually completed within 24 hours; if labs are needed, your timeline depends on when you can get blood drawn (typically a few days at any Quest or Labcorp).
If your physician determines a protocol isn't right for you, you are not charged. We'll explain the reason and (when applicable) point you to alternative protocols you may qualify for. Eligibility is confirmed before any billing.
No. OPTML is cash-pay; we don't bill insurance. All pricing is flat and includes the medication, supplies, and physician oversight. HSA and FSA cards are accepted.
Patients must be 18 or older. Some protocols (e.g., TRT, HRT) have additional clinical criteria your physician will review at intake.

TRT directly replaces testosterone via weekly subcutaneous or intramuscular injection. Fast, predictable, but typically suppresses your body's own production and reduces fertility. Enclomiphene is an oral tablet that stimulates your own testosterone production by acting on the HPG axis. Slower and gentler, but preserves fertility. See the full comparison article.
Both are GLP-1-class medications for weight management. Semaglutide (the active ingredient in Ozempic/Wegovy) acts on GLP-1 receptors and produces about 15% body weight loss at the highest dose. Tirzepatide (the active in Mounjaro/Zepbound) acts on both GLP-1 and GIP receptors and produces roughly 20% weight loss. You complete a ~4-minute medical history and goals questionnaire. A licensed U.S. physician reviews your answers (and any labs). If you're a candidate, the physician designs your protocol, e-signs a prescription, and the compounded medication is dispensed by a 503A pharmacy partner and shipped to you within 2–3 business days.
Physician review is typically within 24 hours of intake submission. From prescription approval to medication delivery is usually 2–3 business days. End-to-end from sign-up to first injection is most commonly 3–5 days when no new labs are needed.
Yes. Unlimited provider messaging is included in every protocol's monthly price. You can ask dose questions, report side effects, request titration changes, etc.
Titration is the gradual adjustment of your dose to the level where you get the best response with the fewest side effects. For GLP-1s, this typically means starting at the lowest dose and incrementally moving up every 4 weeks. Your provider directs each step based on your response and tolerance.
Refills ship automatically each cycle based on your prescription. You don't need to call in or re-submit anything. You can pause, change dose, or cancel from your patient portal anytime.
Each shipment includes pre-filled supplies and a step-by-step injection guide. For GLP-1s the standard route is subcutaneous (under the skin, abdomen or thigh) using a small insulin-grade syringe. Your provider walks you through it; many patients describe it as essentially painless after the first attempt.

Most panels return within 3–5 business days from draw. You'll receive every marker with normal-range context plus a physician-reviewed summary — not just a portal full of numbers.
Often yes — labs within the past 12 months are usually accepted, depending on the protocol. Upload them at intake and your physician will confirm whether new labs are needed.

Depends on the protocol. GLP-1s: nausea, constipation, decreased appetite, fatigue — typically mild and tied to dose titration. TRT: increased hematocrit, acne, possible mood changes. HRT: breast tenderness, breakthrough bleeding (first cycles). Sermorelin: injection-site reactions, occasional drowsiness near bedtime. NAD+: transient warmth or flushing at the injection site. Telegra MD is the U.S. physician-led telehealth services provider that powers OPTML's clinical operations. Licensed physicians within the Telegra MD network conduct intake review, prescribing, and ongoing care.
A 503A pharmacy is a state-licensed, FDA-registered compounding pharmacy that prepares medications for individual patients under valid physician prescriptions, following USP <797> sterility standards. OPTML's compounded medications are dispensed by 503A partners including Strive, Boothwyn, and Absolute.

Free 2–3 business day shipping nationwide. Once your prescription is approved by your physician, your medication leaves the 503A pharmacy in insulated, temperature-controlled, plain packaging.
Yes. All shipments use plain outer packaging with no medical branding visible.
Refrigerate the vial after first reconstitution. Full storage and handling instructions ship with every kit. Each vial is stable for the period specified in your dispensing label.

From your patient portal — there's a cancel option on your subscription page. No call required, no retention script. You can also pause your subscription if you want to take a break without canceling entirely.
Refunds for unshipped medication are issued per OPTML's standard policy. Medication that has already shipped cannot be returned for safety reasons. If you have a billing question, contact info@optml.com.

No. The Assistant does not provide medical advice, diagnosis, or treatment recommendations. All clinical decisions about your care are made by a licensed U.S. physician after individual evaluation. The Assistant exists to help you find information about OPTML's products and process — for personal medical guidance, talk to your provider through the patient portal or call 911 for emergencies.
No. The Assistant does not use a generative language model. It is a curated library of pre-written responses keyed to common topics. This is by design — it ensures answers are accurate, on-brand, and bounded to information OPTML has verified.
