Quick Answer

Testosterone Replacement Therapy (TRT) uses pharmaceutical-grade testosterone, most commonly testosterone cypionate 200 mg/mL, to restore total T to optimal range in men with biochemically confirmed low T. Typical dosing: 80-200 mg weekly via subcutaneous or intramuscular injection, calibrated to total T, free T, SHBG, and estradiol labs. Standard symptoms screened: low energy, low libido, brain fog, poor sleep, and reduced morning erections.

Step 1 of 4 · Choose your path

TRT or enclomiphene? It depends on fertility.

OPTML offers both. Your provider helps you choose based on labs, age, and whether you want to preserve fertility now or later.

Injection

Boosts testosterone directly.

Exogenous testosterone delivered via injection to raise total T levels quickly and predictably.

  • Direct hormone replacement
  • Fastest path to optimal range
  • Suppresses natural T production
  • Not for men planning pregnancy
Oral pill

Stimulates your body's natural T.

Encourages your body to produce its own testosterone while helping preserve fertility.

  • Fertility-preserving
  • Once-daily oral capsule
  • Best for men under 40 or planning to conceive
Option 2 · Fertility-preserving
Enclomiphene · Oral pill

Enclomiphene citrate

Daily oral capsule. Raises your body's own testosterone via the LH/FSH pathway — testicular function and fertility preserved.

  • Total T rises 200-400 ng/dL by weeks 8-12
  • Preserves fertility — sperm production maintained
  • Daily oral capsule — no needles, no cold-chain shipping
  • Best fit for men still planning to have children, or under 40

After you order

Expected time from order to your door.

Your medication is not sitting on a shelf waiting for you. It is prepared for you, to your prescription, after a physician has read your file — and that is the reason for the wait. Here is where every day goes.

7–10 days
from ordering to it arriving, for a straightforward protocol. Roughly 5–7 days for the physician and the pharmacy, then 2–3 days in transit.
Your online visitabout 10 minutes

Done from your kitchen table, not a waiting room. No appointment to book, no clinic to drive to, no time off work. You answer questions about your health and your goals, and that is the whole of your part in it.

A physician actually reads your filethe slow part, on purpose

A U.S.-licensed physician goes through what you submitted, and your lab work where your protocol calls for it. This is a real clinical decision made by a real person, which is why it takes days rather than minutes — and it is the step you would least want us to rush.

Some protocols need a further conversation, or bloodwork first. TRT, enclomiphene and HRT commonly do.

Your medication is made for younot off a shelf

Once a physician approves it, your prescription goes to a state-licensed, FDA-registered 503A compounding pharmacy, where it is prepared for you individually under USP <797> sterility standards.

That is what compounding means: it is made to your prescription after it is written, rather than manufactured in advance and picked from stock. It is also why this is measured in days. Compounded preparations are not FDA-approved.

It ships to your door2–3 days

Insulated and temperature-controlled, in plain packaging with no medical branding on the outside. Shipping is free and already in your price, and tracking is emailed when it leaves.

The one thing that holds an order upworth reading

An order pauses if something is still outstanding on your side — an intake form you have not finished, ID verification, or lab work. It is the most common reason an order goes quiet, and the system cannot always alert you when it happens.

So if it has been quiet for a couple of days, check your email and your patient portal before anything else. That is nearly always where the missing piece is, and clearing it starts your order moving again immediately. Full checklist here.

Ordering on a Friday or at the weekend

Orders are processed seven days a week. What is not covered at weekends is anything needing a person — a stuck form, an ID check, a question for your provider. Order late on a Friday and you will likely hear more at the start of the week.

How to check where you are

Your patient portal shows the live stage of your order. If your purchase appears there and you have the confirmation email, you are in the queue and nothing is wrong.

These are typical ranges, not guarantees, and delays do happen. Something look wrong? Email info@optml.health and a person will pick it up. For anything clinical, message your physician in the portal. For emergencies, call 911.

Frequently asked questions

Yes, TRT requires confirmed low testosterone (typically two morning labs below 300 ng/dL, or symptomatic men with free T in the low quartile). We order a comprehensive panel including total T, free T, estradiol, SHBG, PSA, CBC, and a metabolic panel.
TRT therapy requires ongoing safety monitoring. Your OPTML physician orders baseline labs, reviews them, redraws at week 6 and week 12, and monitors hematocrit, estradiol, lipids, and PSA on an ongoing schedule. Compounded medications are not FDA-approved. Individual results vary.
TRT suppresses the HPG axis and typically reduces sperm production. If fertility preservation is a priority, we offer Enclomiphene, which raises natural testosterone without suppressing fertility, or combination protocols with hCG.
We primarily prescribe testosterone cypionate as a subcutaneous or intramuscular injection, typically weekly or twice-weekly. Some patients prefer creams or pellets; speak with your physician about alternatives.
Common: acne, oily skin, mild fluid retention, mood changes early on. Less common but monitored: elevated hematocrit, high estradiol, worsened sleep apnea. Regular labs let us catch and adjust quickly.
OPTML currently services all 50 states with licensed telehealth physicians. Some states require additional documentation, we'll let you know during intake.

Important safety information

Testosterone is a controlled substance with real contraindications. Read the full eligibility, side effects, and warnings before starting. Not appropriate for men with prostate cancer, untreated severe sleep apnea, uncontrolled heart failure, or hematocrit above the upper reference range.
