Yes, TRT requires confirmed low testosterone (typically two morning labs below 300 ng/dL, or symptomatic men with free T in the low quartile). We order a comprehensive panel including total T, free T, estradiol, SHBG, PSA, CBC, and a metabolic panel.
The TRAVERSE trial (2023), the largest cardiovascular safety study of TRT ever done, found TRT was non-inferior to placebo for major adverse cardiac events in men with hypogonadism. Proper monitoring (hematocrit, lipids, blood pressure) is essential, and OPTML handles all of it.
