Hair that's thinning or shedding

If your hair is thinning at the part, shedding in the shower, or filling the brush, the instinct is to reach for a supplement. The more useful move is to find out why, because the honest research shows most single-nutrient links are debated and supplementing someone who is not deficient rarely helps. Testing tells you which lever is actually yours.

Why hair thins

Hair follicles are among the most metabolically active tissues in the body, cycling constantly between growth and rest. A wide range of stressors, low iron, thyroid trouble, a crash diet, a hormone shift, a serious illness, can knock large numbers of follicles into the resting phase at once, so the shedding you see today often reflects something that happened two or three months ago.

That delay, and the fact that several causes look identical from the outside, is why guessing rarely works. Below, each possible driver is paired with what the research actually found, linked to the study, and flagged honestly. The consistent theme is that many of these links are associational rather than proven, and that the value of a blood test is to find a real, correctable cause, not to justify a generic supplement.

What drives it, and what the evidence shows

1. Iron deficiency and low ferritin

Iron is a cofactor for the enzyme that lets rapidly dividing follicle cells copy their DNA, so low stores can push follicles into shedding early, often before anemia shows up. The link is real but genuinely debated, so it is worth checking rather than assuming.

Serum ferritin was significantly lower in women with androgenetic and areata hair loss, though the same study did not find it in classic telogen effluvium.

2. Thyroid dysfunction, high or low

Thyroid hormones directly regulate how fast follicle cells grow, differentiate, and cycle, so both an underactive and an overactive thyroid can drive diffuse shedding, often months before other thyroid symptoms are obvious.

Hyperthyroidism, hypothyroidism, and drug-induced hypothyroidism can each cause widespread shedding by disrupting follicle growth, differentiation, and metabolism.

3. Rapid weight loss, crash dieting, or low protein

A sudden calorie or protein shortfall is a classic trigger for telogen effluvium: the body deprioritizes non-essential tissue like hair, and a wave of follicles enters the resting phase, shedding two to three months later. Hair shafts are almost entirely protein, so inadequate protein compounds it.

Documented deficiencies should be corrected, but there is no clear evidence that supplementing a person who is not deficient helps, and some supplements can worsen shedding.

4. Vitamin D deficiency

The vitamin D receptor sits inside hair-follicle cells and helps drive the follicle out of its resting phase into a new growth cycle. Low vitamin D is repeatedly found alongside several non-scarring hair-loss types, though whether it is a cause or a bystander is unresolved.

Across more than 3,000 patients, people with non-scarring hair loss had lower vitamin D and roughly three times the odds of deficiency versus controls.

5. Zinc deficiency

Zinc is a cofactor for many follicle enzymes and helps restrain premature follicle regression. Low serum zinc shows up more often in several hair-loss groups, especially alopecia areata and telogen effluvium.

Across 312 people with hair loss, mean serum zinc was significantly lower than controls, with true deficiency clustered in areata and telogen effluvium.

A single-center study with only 30 controls. It shows correlation, not that zinc regrows hair in someone who is not deficient. Copper was not different, so there is no need to chase it.

6. Androgens, PCOS, and female-pattern loss

In genetically susceptible follicles, androgens progressively shrink the follicle and thin the crown or part line. In PCOS, higher testosterone and lower SHBG raise androgen exposure. Importantly, most women with female-pattern thinning have entirely normal blood androgens.

Isolated female-pattern hair loss should not be read as a sign of hormone excess when androgen levels are normal.

7. Vitamin B12 and folate

B12 and folate are essential for the rapid cell division of a growing follicle, so deficiency can contribute to diffuse shedding. Both are cheap, common, and correctable when genuinely low, though they are among the less frequent culprits.

In shedding patients, vitamin D, ferritin, and zinc deficiencies were common enough to justify routine testing, while B12 and folate were abnormal far less often.

8. Biotin, the popular non-answer

Biotin is a genuine cofactor for hair-related enzymes, and true biotin deficiency, which is rare, can cause hair loss. But clinically meaningful deficiency is uncommon on a normal diet, and biotin's fame vastly outruns its evidence. It is the clearest example of why testing beats defaulting to a pill.

The use of biotin as a hair supplement is not supported by high-quality studies and should be reserved for documented deficiency.

How hair thinning is actually evaluated

Because diffuse thinning has several possible drivers that look alike, a sensible first-line workup checks them together. A physician reads ferritin, thyroid, vitamin D, zinc, and B12 in context, and adds androgen markers only when there are signs of hormone excess. A single value rarely tells the story, but a coherent panel usually points to the correctable cause, if there is one.

When to see a doctor

See a physician promptly if your hair loss shows any of the following, since these need timely, sometimes urgent, evaluation:

  • Patchy, coin-shaped bald spots, or a receding band along the front or sides of the hairline
  • Scalp redness, scaling, scarring, loss of visible follicle openings, or itching, burning, or pain
  • Sudden, rapid, large-volume shedding, or hair that breaks off rather than falling from the root
  • Hair loss with fatigue, weight change, cold or heat intolerance, irregular or absent periods, new acne or facial hair, or a facial or body rash
  • Thinning that keeps worsening or fails to recover after about six months, occurs in a child, or follows a new medication

Gradual, diffuse thinning that tracks with a recent stressor, diet change, or family pattern is usually worth investigating with labs first. Patchy loss or a scarred, painful scalp needs prompt dermatology care before follicles are lost for good.

Common questions

Will taking biotin regrow my hair?
Almost certainly not, unless you have a true and rare biotin deficiency. High-quality studies do not support biotin for hair loss in people who are not deficient, and high doses can even distort thyroid and cardiac lab results. Finding the real cause is the better use of your effort.
Is low iron really a cause of hair loss?
It can be, but the evidence is genuinely mixed. Some studies link low ferritin to certain types of hair loss and others do not. That uncertainty is exactly why measuring ferritin, rather than assuming, is the sensible step.
What single blood test explains my hair loss?
There is no single test, because several nutritional and hormonal factors can be involved. The efficient starting point is a panel covering ferritin, thyroid, vitamin D, zinc, and B12, with androgen markers added when signs of hormone excess are present.
Why did my hair start shedding months after a stressful event?
That timing is typical of telogen effluvium. A stressor like illness, surgery, childbirth, or a crash diet pushes many follicles into the resting phase at once, and the visible shedding follows two to three months later.
Should women with hair loss get their hormones checked?
Only when there are other signs of androgen excess, such as irregular periods, cystic acne, or increased facial hair. Most women with female-pattern thinning have normal androgen levels, so hormone testing is targeted, not automatic.
How often should I test?
A baseline is the starting point, then a re-check a few months after correcting any deficiency or making a meaningful change, interpreted with your physician. Because hair responds slowly, the trend over time matters more than one snapshot.
